Registration Information 2020-21

Below please find all registration paperwork that will need to be completed to register your child for the 2020-2021 school year. The student profile sheet will need to be updated with all correct information and the additional registration forms need to be completed.
 
PLEASE NOTE THE PROOF OF RESIDENCY REQUIREMENTS LISTED BELOW.  THESE DOCUMENTS WILL BE NEEDED TO REGISTER YOUR STUDENT.
 
Present ONE of the following items: OR Present TWO of the following items:
* Most recent property tax bill and proof of payment
* Mortgage papers (homeowners)
* Signed and dated lease and receipt of last month’s payment (renters) 
 
* Voter registration
 * Vehicle registration
* Most recent gas, electric, and/or water bill, within the last 30 days
 * Most recent cable bill
* Most recent credit card bill
* Current public aide card
* Current homeowner/renters insurance policy
 
If you are unable to provide these residency requirements please call the district office at 815-389-3478 and ask for Lisa to set up an affidavit of residency appointment.

Open registration for the 2020-2021 school year will be held at each individual school during the following dates and times:
 
Monday June 22nd-Friday June 26, 2020 10 a.m.  –   5:30 p.m.
For High School Only:
Registration is by grade per day
Monday June 22nd  - 9th
Tuesday June 23rd - 10th
Wednesday June 24th - 11th
Thursday June 25 - 12th
Friday June 26th - New students and any that could not make it on the specific day. 
10 a.m. - 5:30 p.m.
 

If you are unable to make any of these registration times, you must contact your child’s school for late registration. 

Registration packets did go out the first week of June, if you did not receive one please contact your child's school.  

There will be no increase in fees for 2020-2020, please see the Fee Tally Sheet sample for details.  

Our school is participating in the Community Eligibility Provision (CEP) under the National School Lunch Program. Under CEP, all students receive a breakfast/lunch at no charge for the entire school year.

The District requires all families to complete a Household Income Form to determine eligibility for registration fees.  If you do not wish to apply for benefits, please complete Section 1 of your child/s name, school and grade, write “decline” in Section 3 and please sign Section 4.
