Dental Requirements

Dental exams:
-  Kindergarten, 2nd, and 6th grades: Oral health exam is required before May 15, 2020.
 
Objections: A child’s parent or legal guardian who objects to health or dental examinations or immunizations
must submit an Illinois Certificate of Religious Exemptions form. For the correct form required by the state,
please contact your health care provider.

Vision Requirements
 
Vision exams:
- Kindergarten: Students entering kindergarten must have an eye exam.

Hearing Requirements
 
The Illinois Department of Public Health (IDPH) requires the following screenings for students in grades Pre-K, K, 1, and 2:
 
1. All Pre-K students need hearing and vision screenings conducted.
2. All K students need hearing screenings conducted. Vision screens will be conducted on those students who
have not turned in a recent vision exam to the nurse.
3. All 1st graders will have a hearing screening conducted.
4. All 2nd graders will have both hearing and vision screens conducted.
5. Any new student to the district needs both hearing and vision screens conducted.
6. Any student that is in the special education program should have hearing and vision screened annually.
7. Any teacher referrals will have the appropriate screening.

*Any student that has an eye exam within the past 12 months on file or actively sees an eye doctor will not be
screened.

Please note: Any new student to the state of Illinois must have an eye exam by a licensed optometrist or
ophthalmologist by an Illinois physician.
