Health Emergency Action Plans
The State of Illinois has passed legislation requiring all school districts to request action plans for all students diagnosed with certain diseases. The laws state that the action plan must be requested from the parent/guardian of the student every school year. As a result of your student being diagnosed with asthma we are in need of an asthma action plan that has been developed by your student’s treating physician. We have attached additional action plans that we encourage you to use if your student is diagnosed with a severe allergy or diabetes. These plans will assist our staff in the event of an emergency with your student.
