Physicals and Immunizations
Please be advised of the following physical examination and immunization requirements per the State of Illinois and South Beloit Community Unit School District #320.
The School Code of Illinois (Section 27-8.1) requires the following:
Pre-K: Physical exam on Illinois physical form, & up to date immunization records
Kindergarten: Physical exam on Illinois physical form, up to date immunization records, dental exam, & vision exam
2nd grade: Dental exam
6th Grade: Physical exam on Illinois physical form, Tdap booster, Meningitis vaccine & dental exam
9th Grade: Physical exam on Illinois physical form & dental exam
12th Grade: 2nd meningitis vaccine
Students entering an Illinois school for the first time: Physical exam completed within the last year on Illinois physical form, up to date immunization records, dental exam, & vision exam
You can have this completed at Crusader Clinic located in the South Beloit High School or with your primary physician. This needs to be completed and turned in before October 15th or your child may be excluded from attending school. If this has already been completed, please provide a copy.
Objections: A child's parent or legal guardian who objects to health/dental/eye examinations OR immunizations must submit an Illinois Certificate of Religious Exemptions form. If the child has a medical condition exempting them from immunizations, a statement from their Physician is required. For the correct form required by the state, please contact your health care provider.
